- Send a $50 Application Fee to:

4. Print off Form # 4 (Character Reference) found under Prospective Students/Application Packet and give to an
adult Christian friend, age 21 or over to fill out and submit. Make sure you complete the WAIVER STATEMENT
and sign it before giving it to your character reference.
5. Print off Form #5 (Transcript Request) and send to each previous school's office.
6. Print off Form #6 (Medical Form). Fill out form and sign. Mail or fax to IBC. Fax: 1-306-692-7968
